报告摘要:
Human papillomavirus (HPV) is a sexually transmitted virus that affects a large fraction of the global population at any time. Most infected individuals are asymptomatic; however, some of the infections result in cervical cancer in women and throat cancer in men. The infection can be effectively prevented by HPV vaccine but who should be vaccinated and how to best allocate available vaccines remain unclear. Using mathematical modeling and computer simulations, our results suggest that to eliminate HPV infection the priority should be given to MSM (men who have sex with men) in addition to vaccinating heterosexual females or males. The best vaccination strategy is to vaccinate MSM firstly as many as possible, then heterosexual females, lastly heterosexual males. Heterosexual males and females should not be vaccinated at an equal percentage as that will increase the HPV prevalence in the total population. The evener the distribution, the higher the disease prevalence. Vaccination also becomes less effective in reducing HPV prevalence as more vaccines are given. This study provides quantitative information that may help policymakers formulate guidelines for HPV vaccine distribution on the basis of vaccine availability and prior vaccination coverage.
